The RPN Flaw / Trap

A mathematical limitation of RPN where a catastrophic life-safety failure (S=10) yields a deceptively low score if frequency is rare (O=1, D=1 → RPN=10). SMRP strictly mandates mitigating high severity first regardless of overall RPN.

The golden governance rule of SMRP: exactly one individual must be assigned Accountable (A) per task. If accountability is shared or absent, ownership evaporates and governance collapses.

Centralized vs Decentralized Maintenance

Modifying leading indicators directly shapes future results.

Lagging Indicators (Outcome Metrics)

Performance metrics that measure bottom-line historical results and outcomes after events occur, which cannot be changed retroactively (e.g., unplanned downtime, total maintenance cost, TRIFR safety rate, emergency work orders). They serve as a rearview mirror evaluating the effectiveness of prior leading activities.

Mean Time Between Failures (MTBF)

Mean Time to Repair (MTTR)

Calculated by dividing total repair downtime hours by the number of repairs: MTTR = (Total Repair Downtime / Number of Repairs). Lower MTTR values reflect available job packages, kitted spare parts, and technician expertise.

Operational Availability (Ao)

The percentage of time an asset is in an operable condition capable of production relative to required operating time. Governed by the classical formulation: Availability = MTBF / (MTBF + MTTR). Availability increases by boosting MTBF or driving down MTTR.

Management of Change (MOC)
