مشهد مشترك في كل مبنى: حفرة تخديم مفتوحة بجوار ممر يعبره الناس، وضوضاء مولد تتجاوز الحد المسموح صباح كل يوم. السؤال الذي يسبق أي إجراء: أيهما أهم أولاً، وأي حل نختار؟ هذا الدرس يمنحك أداة الإجابة: طريقة منظمة لتسجيل المخاطر، وترتيبها، واختيار السيطرة.

1. الخطر والمخاطرة — بابان يجب الفصل بينهما

  • الخطر (Hazard): مصدر الضرر المحتمل — كابل مكشوف، أرضية زلقة، غاز قابل للاشتعال.
  • المخاطرة (Risk): مقياس الضرر المحتمل من هذا الخطر، ويساوي الاحتمال × الشدة.

الفرق العملي: الخطر قد يبقى موجوداً (الكابل مكشوف)، لكن المخاطرة يمكن خفضها — بتقليل احتمال التعرض (فصل الكهرباء قبل الصيانة) أو تقليل الأثر (درع وقاية، مسافة آمنة، تدريب).

2. خطوات تقييم المخاطر

  1. التحديد (Identification): حدد المخاطر في الموقع — جولة ميدانية، سجلات الحوادث السابقة، مقابلات مع العاملين، قوائم تحقق معيارية.
  2. التحليل (Analysis): لكل خطر عيّن الاحتمال والشدة واحسب الدرجة.
  3. التقييم والترتيب (Evaluation): عالج أولاً ما يحمل أعلى درجة.
  4. السيطرة (Control): طبّق أسلوباً من هرم التحكم (القسم 4).
  5. المراجعة (Review): أعد القياس بعد التغييرات، وبعد الحوادث، وسنوياً على الأقل.

3. مصفوفة المخاطر 5×5 (Risk Matrix): الاحتمال × الشدة

تُقيَّم الأبعاد بالأرقام من 1 إلى 5:

  • الاحتمال (Likelihood): 1 نادر → 5 شبه مؤكد.
  • الشدة (Severity): 1 تافهة → 5 كارثية.

والدرجة = الاحتمال × الشدة، وتُصنَّف إلى نطاقات:

مثال تطبيقي:

  • حفرة بجوار ممر مشاة: احتمال التعرض (4)، الشدة (4) → الدرجة 16 — حرج: لا تنتظر.
  • ضوضاء غرفة مولدات: احتمال التعرض (5)، الشدة (3) → الدرجة 15 — حرج.
  • سلم مكسور في مخزن نادر الاستخدام: احتمال (2)، الشدة (3) → الدرجة 6 — متوسط.
⚠️ احذر Be careful

مفهوم خاطئ: «المخاطر النادرة لا تستحق الاهتمام». الحقيقة: التقييم حاصل ضرب، لا جمع. خطر نادر لكن شدته كارثية (انفجار، حريق كبير) قد يحمل درجة أعلى من خطر يومي تافه. لا تحكم من شيوعه وحده.

4. هرم السيطرة (Hierarchy of Controls): من الأقوى إلى الأضعف

عندما تقرر «كيف نتدخل»، تسير من أعلى الهرم نحو الأسفل، وتختار أعلى وسيلة قابلة للتطبيق:

  1. الإزالة (Elimination): أزل الخطر من الأساس. مثال: إلغاء مخزن مواد قابلة للاشتعال داخل المبنى.
  2. الاستبدال (Substitution): بديل أقل خطراً. مثال: دهان مائي بدل دهان يطلق أبخرة سامة.
  3. السيطرة الهندسية (Engineering): افصل الناس بالتصميم. مثال: عازل صوتي حول مولد، غطاء متين للحفرة، سور أمان.
  4. السيطرة الإدارية (Administrative): نظّم السلوك. مثال: منع المرور مؤقتاً، لافتة، تدريب، تقصير زمن التعرض.
  5. معدات الوقاية الشخصية (PPE): آخر خط دفاع. مثال: سدادات أذن خلال الدخول المؤقت للغرفة.

السبب في هذا الترتيب أن الأبعد (PPE) يعتمد على سلوك الفرد، بينما الهندسة تحمي الجميع من دون اشتراك أحد.

تُسير من قمة الهرم نحو قاعدته وتختار أعلى وسيلة قابلة للتطبيق: الأبعد (PPE) يعتمد على سلوك الفرد بينما الهندسة تحمي الجميع.

5. متى يُعاد تقييم المخاطر؟

لا يُجرى التقييم مرة واحدة ويُنسى؛ يُعاد:

  • بعد أي تغيير في المعدات أو العمليات أو المواد.
  • بعد كل حادث أو شبه حادث (Near Miss).
  • سنوياً على الأقل، أو عند تغير المعايير القانونية.

مثال: غطيت الحفرة من الشهر الماضي، فعادت الدرجة من 16 إلى 3 — التقييم يثبت أن القرار الهندسي فعّال، ولا يترك مجالاً للاعتماد على «النية».

A common scene in every building: an open service pit beside a corridor people cross, and generator noise exceeding the allowed limit every morning. The question that precedes any action: which matters first, and which solution do we choose? This lesson gives you the answer tool: an organized method to record risks, rank them, and choose the control.

1. Hazard and risk — two doors that must be kept apart

  • Hazard: the source of potential harm — an exposed cable, a slippery floor, a flammable gas.
  • Risk: the measure of potential harm from that hazard, equal to likelihood × severity.

The practical difference: the hazard may remain present (the cable exposed), but the risk can be reduced — by lowering the likelihood of exposure (isolating power before maintenance) or by reducing the impact (a protective shield, a safe distance, training).

2. The risk assessment steps

  1. Identification: identify the hazards on site — a field walk, past incident records, interviews with workers, standard checklists.
  2. Analysis: for each hazard assign likelihood and severity and compute the score.
  3. Evaluation and ranking: treat first what carries the highest score.
  4. Control: apply a method from the hierarchy of controls (section 4).
  5. Review: re-measure after changes, after incidents, and at least annually.

3. The 5×5 Risk Matrix: likelihood × severity

The dimensions are scored from 1 to 5:

  • Likelihood: 1 rare → 5 almost certain.
  • Severity: 1 trivial → 5 catastrophic.

The score = likelihood × severity, classified into ranges:

Worked example:

  • A pit beside a pedestrian walkway: likelihood of exposure (4), severity (4) → score 16 — critical: do not wait.
  • Generator room noise: likelihood of exposure (5), severity (3) → score 15 — critical.
  • A broken ladder in a rarely used store: likelihood (2), severity (3) → score 6 — medium.

Misconception: “Rare risks do not deserve attention.” Fact: the assessment is a product, not a sum. A rare hazard whose severity is catastrophic (an explosion, a major fire) may carry a higher score than a trivial daily one. Do not judge from its frequency alone.

4. The Hierarchy of Controls: from strongest to weakest

When you decide “how do we intervene”, you move from the top of the hierarchy down, choosing the highest applicable measure:

  1. Elimination: remove the hazard at its root. Example: removing the flammable materials store inside the building.
  2. Substitution: a less hazardous alternative. Example: water-based paint instead of paint releasing toxic fumes.
  3. Engineering: separate people by design. Example: an acoustic enclosure around a generator, a sturdy pit cover, a safety railing.
  4. Administrative: organize behavior. Example: temporarily banning passage, a sign, training, shortening exposure time.
  5. Personal Protective Equipment (PPE): the last line of defense. Example: earplugs during temporary entry into the room.

The reason for this order is that the lowest (PPE) depends on individual behavior, while engineering protects everyone without anyone’s participation.

You move from the top of the hierarchy toward its base and choose the highest applicable measure: the lowest (PPE) depends on individual behavior while engineering protects everyone.

5. When is risk assessment repeated?

The assessment is not done once and forgotten; it is repeated:

  • After any change in equipment, processes, or materials.
  • After every incident or near miss.
  • At least annually, or when legal standards change.

Example: you covered the pit last month, so the score returned from 16 to 3 — the assessment proves that the engineering decision is effective and leaves no room for relying on “intention”.
