مفهوم خاطئ: «توزيع أقنعة التنفس هو الحل الأسرع والأرخص لأبخرة المذيبات.» الحقيقة: القناع قاع الهرم وأضعف خطوط الدفاع؛ ابدأ باستبدال المذيب السام بمنظف آمن أو تركيب كابينة شفط مغلقة، ولا تلجأ للوقاية الشخصية إلا حلاً مؤقتاً أو أخيراً.

2. مصفوفة تقييم المخاطر ومبدأ ALARP

درجة الخطر = احتمالية الحدوث (Likelihood) × شدة العواقب (Severity). وحتى الاحتمالية المنخفضة مع عواقب كارثية — كتسرب غاز من مانع تسرب مضخة وقود يحدث مرة كل 5 سنوات لكنه يسبب انفجاراً — تصنف خطراً حرجاً أحمر لا يقبل التشغيل حتى الانهيار، بل يستلزم تحكماً هندسياً صارماً كمانع مزدوج ومراقبة آلية وتفتيش تنبؤي.

مبدأ ALARP (أقل مستوى ممكن عملياً) يحكم إدارة الخطر المتوسط: تخفض الخطر إلى أدنى حد ممكن عملياً، ولا تقبل أبداً خطراً يهدد الأرواح. والنتيجة ثلاث مناطق: منخفض مقبول يدار بالإجراءات القياسية، ومتوسط يدار بالتفتيش التنبؤي وتحليل سلامة العمل وعزل الطاقة، وحرج يحظر التشغيل حتى التحكم الهندسي.

3. نظام إدارة التغيير (MOC) وإدارة سلامة العمليات (PSM)

إدارة التغيير (Management of Change) إجراء إلزامي بموجب OSHA 1910.119 لمنع كوارث نشأت تاريخياً عن تعديلات حسنة النية دون دراسة — ويفرق بين حالتين:

  • الاستبدال المطابق للأصل (Replacement in Kind): قطعة تطابق التصميم الأصلي 100% في الخامة والأبعاد والضغط والقدرة — لا يحتاج دورة MOC ويُغلق بأمر شغل عادي.

4. عزل الطاقة (LOTO) وتصريح العمل (PTW) وتحليل سلامة العمل (JSA)

تحليل سلامة العمل (Job Safety Analysis) تفكيك المهمة إلى خطوات وتحديد مخاطر كل خطوة ووضع ضوابطها قبل البدء، ومن أهم ضوابطه عزل الطاقة. وقواعد عزل الطاقة (Lockout Tagout) صارمة بلا تهاون:

  • قفل لكل فرد: كل فني يضع قفله الشخصي بمفتاحه الخاص، ولا ينزع أحد قفل غيره إلا بإجراء طوارئ معتمد.

Asset reliability is the first line of defense for lives and the environment: a neglected safety valve or a hasty field modification can turn into an explosion that destroys the plant. This lesson places you in front of your gravest responsibility — how you control a hazard from its summit rather than its base, and how you govern any change before it becomes a catastrophe.

1. The Hierarchy of Controls

The Hierarchy of Controls is the order adopted in ISO 45001, from most effective to least effective, and the most famous mistake is starting from the bottom by handing out protective equipment:

  • Elimination: removing the hazard source entirely, such as automating tank cleaning with robots to keep people out of confined spaces — full effectiveness.
  • Substitution: replacing the hazardous substance with a safe alternative, such as a bio water-based solvent instead of the toxic chlorinated one — high effectiveness.
  • Engineering controls: isolating the hazard with physical barriers, such as belt guards, interlocked switches, and local exhaust hoods — a sustainable solution.
  • Administrative controls: changing work behavior, such as work permits, training, and technician rotation — depends on compliance.
  • Personal Protective Equipment (PPE): helmets, goggles, and respirators — the last line of defense, which does not eliminate the hazard but only reduces the injury.

Misconception: “Handing out respirators is the fastest and cheapest solution for solvent vapors.” Fact: the mask is the base of the pyramid and the weakest line of defense; start by replacing the toxic solvent with a safe cleaner or installing an enclosed extraction booth, and resort to PPE only as a temporary or final solution.

2. The risk assessment matrix and the ALARP principle

Risk score = Likelihood × Severity. Even low likelihood combined with catastrophic consequences — such as a gas leak from a fuel pump mechanical seal that happens once every 5 years but causes an explosion — is classified as a critical red risk that does not tolerate run-to-failure; it requires strict engineering control such as a double seal, automatic monitoring, and predictive inspection.

The ALARP principle (As Low As Reasonably Practicable) governs medium-risk management: you lower the risk to the lowest level reasonably practicable, and you never accept a risk that threatens lives. The result is three zones: low and acceptable, managed with standard procedures; medium, managed with predictive inspection, job safety analysis, and energy isolation; and critical, where operation is prohibited until engineering control is in place.

3. Management of Change (MOC) and Process Safety Management (PSM)

Management of Change (MOC) is a mandatory procedure under OSHA 1910.119 to prevent disasters historically caused by well-intentioned modifications made without study — and it distinguishes two cases:

  • Replacement in Kind: a part that matches the original design 100% in material, dimensions, pressure, and capacity — it needs no MOC cycle and is closed with a normal work order.
  • An engineering or operational modification: any difference in valve type, material, motor capacity, piping route, or control logic — prohibited without a full MOC cycle: engineering review and materials compatibility, safety risk assessment, departmental approval, updating P&ID drawings, and staff training.

The Management of Change (MOC) decision tree

    A new part, an engineering modification, or an operational change.

    A matching replacement in material, dimensions, pressure, and capacity (Replacement in Kind).

    Yes — a normal work order

    A matching replacement needs no MOC cycle and is closed with a normal work order.

    No — a full MOC cycle

    Engineering review and materials compatibility, safety risk assessment, departmental approval, updating P&ID drawings, and staff training.

  5. ⚠️

    Execution without the cycle is prohibited

    Well-intentioned modifications made without study were the source of historical disasters under OSHA 1910.119.

Any difference in type, material, capacity, route, or control logic requires a full MOC cycle before execution.

Process Safety Management (PSM) is the broader framework for preventing catastrophic releases of toxic and highly flammable substances, and the reliability engineer is a pillar within it through the integrity of vessels, piping, and relief valves.

The valve rule: replacing a failed ball valve with an available butterfly valve “of the same diameter” is not a matching replacement — type, flow, pressure resistance, and leakage differ, and any trial installation on an ammonia line is a deadly risk. Open an MOC request first.

4. Lockout/Tagout (LOTO), Permit to Work (PTW), and Job Safety Analysis (JSA)

Job Safety Analysis breaks the task into steps, identifies the hazards of each step, and sets their controls before starting; one of its most important controls is energy isolation. Lockout/Tagout (LOTO) rules are strict with zero tolerance:

  • One lock per person: every technician applies their personal lock with their own key, and no one removes another person’s lock except through an approved emergency procedure.
  • Draining residual energy: disconnecting electricity is not enough — vent hydraulics and air, drain capacitors, and secure suspended loads.
  • Verification and testing: attempt a start to confirm no response, and verify zero voltage before touching (Test before Touch), tying work order closure to the approved work permit.

Relying on a verbal assurance that the breaker is open — as in the 11kV panel tragedy — is a fatal violation; and punishing individuals without fixing the system guarantees the incident will recur.
