Replacing the broken part brings the machine back to work today, but it does not prevent the next failure. Root Cause Analysis (RCA) is a structured methodology that penetrates surface symptoms to the cause which, once treated, would eliminate the failure from the entire plant. This lesson explains the cause levels, investigation tools, launch rules, and institutionalization.

1. The three cause levels: where does the real root lie?

Maintenance’s biggest trap is stopping at symptom treatment. Any industrial failure divides into three successive levels:

  • Physical Cause: what happened physically to the metal? Example: a pump shaft broke under violent torsion, or a bearing seized from friction and dried grease. Knowing it alone and treating it by replacing the part guarantees failure recurrence within weeks.
  • Human Cause: what wrong action occurred? Example: the technician did not perform laser alignment, or applied grease that does not meet specifications. Professional note: blaming and reprimanding individuals is a rejected practice that hides the real systemic causes.
  • Latent Cause: why did the human error happen? Example: the laser alignment device is unavailable, there are no written Standard Operating Procedures (SOPs), and no crew training. This is the true target of the investigation; treating it prevents the failure across the entire facility, not just on one machine.

2. Investigation tools: the fishbone and the five whys

For an engineering brainstorming session with a multidisciplinary team, two complementary tools are used:

Ishikawa diagram / Fishbone: a visual tool that collects all probable causes under six comprehensive categories (6Ms) that ensure no possibility is overlooked: Machine (design and component defects), Man (skills, training, and stress), Material (quality of oils, spare parts, and raw materials), Method (work instructions and maintenance cards), Measurement (sensor calibration and inspection instruments), and Milieu (heat, humidity, and dust).

The 5-Whys technique: the famous deep-dive method of repeating the question «Why?» until reaching the administrative system that can be corrected. Example of a pump whose shaft broke: why did it stop? Because the bearing seized from friction. Why? Because of insufficient grease. Why? Because the technician did not pump grease on the preventive round. Why? Because the lubrication point is hidden behind the cover and no written procedure exists. Why are there no procedures? Because management has no planning department and no policy for Job Packages. The final result is a latent organizational cause, and solving it protects the entire plant.

3. When do we investigate? And how do we institutionalize the lesson?

Launching a root-cause investigation for every broken bolt exhausts the team, causes Analysis Paralysis, and produces superficial reports. Best practice requires declared Trigger Criteria, such as: any incident affecting personnel safety or causing a serious environmental violation, any failure whose losses exceed a known financial threshold (such as $10,000), or any failure recurring 3 times within a month on a critical tier-1 asset.

The investigation does not end with writing the report but begins after it, through Institutionalization: updating standard operating procedures (SOPs), modifying preventive maintenance cards in the Computerized Maintenance Management System (CMMS), publishing Lessons Learned, and applying the solutions to all similar machines in the plant. An investigation locked in a drawer is wasted effort with zero return.

This integrates with Continuous Improvement and the Lean approach, which targets Waste in its forms: overproduction beyond need, waiting, piled-up inventory, defective products, and unnecessary motion and transport.
